Nurse

The Nurse is responsible for the daily management of the residential patients with record at emergency care service center. Nurse is responsible for providing quality nursing care and support to individuals with mental health challenges within a holistic setting. This role involves assessment, medication management, therapeutic interventions, and collaboration with a multidisciplinary team to promote the overall well-being and recovery of patients.

MAJOR DUTIES & RESPONSIBILITIES

ESSENTIAL DUTIES & RESPONSIBILITIES

1. Clinical Engagement

  • Conduct comprehensive physical and mental health assessments of patients upon admission and throughout their residential service period.
  • Take part in Mental Status Examination (MSE) of all the beneficiaries in supervision of Psychologists.
  • Record patient vital signs and medical information.
  • Administering and monitoring medication, including psychotropic and other medications, as prescribed by physicians to beneficiaries during rescue, residential, reintegration.
  • Engage beneficiaries in recreational activities at least for an hour a day.
  • Develop and implement individualized treatment plans, Psychiatric care plan in collaboration with Psychologists, Outreach officer, and team.
  • Maintain diet chart of beneficiary as per required and focus for requirement as per diagnosis and health condition.
  • Support Nursing team, Psychologists, Outreach team in developing and implementing individualized care plans based on patients’ needs, preferences, and available resources.

2. Field Engagement

  • Responsible for medication, hygiene and safety of the patient rescue and reintegration from any part of Nepal to KOSHISH’s Emergency Care Service Centre and vice/verse.
  • Responsible for patient Intake and discharge of the patient with official procedure online.
  • Responsible for patient history completion in given format.
  • Coordinate with caregivers to remove potentially harmful and triggering environmental features.
  • Educating the patient and family members about mental health conditions and family support for recovery.
  • Develop community awareness materials to conduct psychoeducation particularly within patient’s family, community, peer support, mothers, self-help groups, based on Mental health condition and different psychosocial issues.

3. Care Coordination

  • Arrange and Maintain appointment schedule of reintegrated beneficiaries in follow up visit at based on follow up intervention by Outreach team.
  • Provide emotional support, and information on Psychotropic medication to patients and their families/guardian.
  • Facilitate activities such as art therapy, gardening, dance session, group discussions, and Agriculture sessions that align with the beneficiaries’ rehabilitation goals.
  • Organize weekly home calls to residential beneficiaries as per the schedule with Outreach team.
  • Provide physical support for patients with daily activities, and personal hygiene including bathing, dressing, exercising.
  • Facilitate group therapy sessions and psychoeducation programs to visitors as required.
  • Collaborate with psychiatrists, psychologists, social workers, and other healthcare professionals including physician, therapists, to coordinate and integrate comprehensive care.
  • Participate in case conferences and treatment planning meetings with psychologist.

4. Safety and safeguarding

  • Ensure patients do not harm themselves or others with any means.
  • Ensure that patients/family/guardian are informed about the treatment and actively participate in decision-making processes.
  • Ensure Patients coordination with in person, through CCTV especially at night shifts.
  • Provide strict monitoring and maintain safety of self and beneficiary with violent nature.
  • Implement infection control measures to ensure safe and hygienic environment.

5. Documentation and Reporting

  • Maintain accurate and up-to-date electronic health records, including assessment findings, care plans, and progress notes.
  • Maintain records of patient history, condition, treatments, and progress.
  • Maintain Medicine records in coordination with Pharmacists.
  • Prepare and provide nursing reports, event reports as required for official procedure.
  • Update patient’s clinical records with details.
  • Arranges further medical attention adhering to professional standards, code of ethics, and organizational policies and procedures when necessary.

ESSENTIAL QUALITIES and SKILLS

  • Registered Nurse (RN) with a valid license.
  • Knowledge of psychiatric medication and their effects.
  • Basic knowledge on MS office application
  • Strong assessment and critical thinking skills.
  • Excellent communication and interpersonal skills
  • Flexible with full time duty on rotating shifts, weekends, public holidays, on call rotation during emergency situation.
  • Competent with exposure to emotionally challenging situations.
  • Adherence to infection control and safety protocols.

Salary: Negotiable as per competence and experience
